Where do i go to print out the DOL-N4 Forms to submit to the Dept of Labor??

Let’s go to your Payroll Tax Center to print out your DOL-N4 forms. I'm here to guide you how.

  1. Go to Taxes from the left menu.
  2. Select the Payroll Tax tab.
  3. Choose Quarterly Forms.
  4. Select GA DOL-4.
  5. Choose the period from the drop-down.
  6. Click View.
  7. Open the form as PDF to print it out.

I’ve attached screenshots below to show you the steps.

  5. Be encouraging and positive. Look for ways to eliminate uncertainty by anticipating people's concerns. Make it apparent that we really like helping them achieve positive outcomes.
