My client received a W-2 from the insurance company on the disability payment received. However another insurance company who paid them earlier have never send the w-2. The insurance premium is paid by the employee.

Could one of the payments received be from the employer's policy and one from the employee's policy? If this is the case, the one from the policy purchased by the employee is not taxable, the one paid by the employer's policy is taxable.
The employee paid full premium out of post tax salary. Initially an insurance company was making payment and after three months the second company came into picture. I presume they would be a reinsurer.

I'd guess short term disability switched over to long-term disability at the 3-month mark, and that Charlotte's analysis is correct.
