I'll bet you can find them on an HR site on the internet. I would recommend that there be a disclaimer that states the employer had permission to debit the account to make corrections in case of a deposit error. Attached is a generic copy of what I use. Check to verify that this doesn't violate any of your state laws. Hope this works for you.

And don't do what I did:
let the employee hand write the info and NOT attach a voided check. He wrote a wrong digit, I had no way of double checking, and it was expensive and took two weeks to get the money back.
Now I insist they attach a voided check or deposit slip - so that any mistake is mine, and I don't have to chase the employee for error fees....
Can you tell me what steps you had to take to get the money back from QB. I'm in a similar situation. The money has been deducted from my payroll account, but was not deposited in the employees account because of an error with his bank account number.

It was quite a while ago, so I don't remember the exact steps. Only that QB uses a third party bank, so the money goes from Company Bank to QB Bank to Employee's Bank.
I looked up "direct deposit" in the help section, and under "voiding" (Which I know is not what you're doing, but close enough to get the info), there are instructions on reversing a DD. I copied and pasted below.
By the way, you should go into your own QB help file, because the email for qb support is highlighted, so you will be able to send an email request directly to them. They will then send you the form to fill out - remember to fill in the incorrect account number so that they deduct it from the right one. As they state over and over, if the money ain't there (the lucky party notices and spends it) then you're out of luck, in addition to the extortion fee of $50.

After a direct deposit is transmitted to an employee's financial institution, there is no way to prevent the funds from being credited to the employee's account. Therefore, your best option is to send the void before the start of payroll processing, which begins at 5:00 p.m. (pacific time), two banking days prior to the paycheck date or on the day we receive your payroll data, whichever is earlier.
If you send the void after your payroll is processed, you can initiate a separate process to reverse the direct deposit as long as you submit the request within three banking days of the deposit date. Enter your request at QuickBooks support. You will receive an e-mail confirmation of your request when we initiate the reversal process with the banking institution. The process can take up to 10 days. We will notify you by e-mail to let you know whether or not the funds were recovered.
Please be aware of the following:
Intuit cannot guarantee that the funds will be recovered.
If the entire amount of the direct deposit is not available in the employee's account, the reversal will be rejected and no funds will be recovered
You will be charged a $50 Direct Deposit Reversal fee, whether or not the funds are recovered.
You always have the option of attempting to recover the funds directly from the employee.
