DE 34 form is blank - should show new employee info
I just added a new employee, effective 4/15/08, and am, for the first time, using the Payroll Form function to generate a CA state DE 34 "Report of New Employee(s)". The form is there to select, and asks for a date range. I've tried several, including just the date of hire, the whole month of April... (this form is due 20 days after start of work for a new employee).The form comes up, populated with my employer information, but it is blank in the areas where the new employee information - name, SSN, address, etc., is required. What am I doing wrong?
