You can attach a pdf file to a business return, not to an individual return. I do not know the answer to the question about the Notes.
I do know about notes. Between the 2 of us, we're answering the original question.
Statement notes get transmitted to IRS electronically, so they can be used as any kind of disclosure statement you want. States, unfortunately, do not get notes transmitted electronically -- not even "statement" notes.
