Legal disclaimer

I have a client that is not giving me all of her information that I need to make a completely accurate file. She doesn't want me to handle her personal accounting, only business, but she is mixing them together. I want to protect myself in a situation like this with a disclaimer, and I was hoping to find a "cookie-cutter" legal disclaimer to have my clients sign that says:1) The information that I process is as accurate as what is given to me. 2) I will not sell/disclose any of her information3) I am not responsible for maintaining physical recordsetc.Is there anything like this available anywhere that is already written?

My best disclaimer for this client would be "I am sorry but I am unable to help you at this time."
